Most elderly people will state
emphatically that they want to stay in their homes as long as
possible to maintain their independence, pride and dignity.
There is a strong emotional attachment as this is where families
were raised and life is shared with neighbors. The appreciated
equity in the house was intended for family, not care
facilities.
Health@Home™ is a personalized,
cost-effective Remote Monitoring and TeleConsultation program
for seniors who now require more attention to their health,
yet are not ready to move out of their home and into an extended
care facility. Seniors can continue to enjoy the freedom of
living at home, and relatives can feel more secure knowing that
a comprehensive array of nursing and care giving resources are
available through the Health@Home™ program.
Our care models begin with the needs of the
patient. A typical patient's health has recently declined
and they now will require additional assistance to continue to
live at home. The patient may be diagnosed with a chronic
illness that requires frequent health monitoring. The
patient may need additional assistance but the move to an
assisted living facility is too expensive, too far away, or too
big a jump. A Personal Care Manager (PCM)
will consult with the patient, the family, the payor and the
physician to implement an individual plan to monitor and assist
the patient in their own home. Our Personal Care Managers
are highly qualified, licensed nursing professionals with home
care experience. The Personal Care
Manager is responsible for the daily monitoring of the patient
and will facilitate communications between all parties involved
in providing in home care. Home Care
Technologies continually monitors the patient in the home and patients
have round the clock access to qualified staff manning our
TeleNursing Control Center in case of emergencies. The Personal Care Manager
has access to monitoring data and can effectively relay the
patients health status to a physician, care giver or family
member. Through regularly scheduled video based
TeleConsultation sessions our monitoring personnel develop a
personal relationship with the patient. In addition,
family members can call the nursing center to talk to the PCM
about their loved one's health.
The personal care model is holistic, proactive
and dynamic. The active collection of monitoring data
allows Home Care Technologies to predict and prevent changes in
the patients health state. The active collection of
accurate health data allows a Personal Care Manager to send a
complete health profile to a physician to better diagnose and
prescribe new care. The interpersonal relationship
developed through video consultation and home visits lift the
emotional state of the patient. The patient looks forward
to these interactions and becomes an active participant in their own care. The
combination of good data, good communications and personal
contact lead to prolonged health for the patient. |
