| Independent Care™ is an integrated
approach to providing personalized health management services to those people who need
it most. This is a program for those diagnosed with a
chronic illness who with additional monitoring and assistance
can maintain an independent healthier life at home. By
enabling patients to receive care in their own homes, more
costly institutional care may be avoided.
Independent Care™ focuses on the top 10% - 20% of
patients who are responsible for a disproportionate share of
healthcare expenses and resources. By implementing a
targeted Independent Care™ program, these patients can live
healthier lives while reducing the overall cost burden on
private and public payors.
Our comprehensive program consists
of all of the critical elements of successful health management,
from identification of at-risk patients to in-depth education,
clinical interaction and compliance management, and quantified
outcomes reporting.

Our dynamic
experience-based personal health management programs provide
individual protocols which adapt to the changing needs of the
patient. Our active monitoring capabilities provide
constant feedback to help manage and track the patients'
outcomes. Our Personal Care Manager oversees the daily
monitoring of the patient. Physicians are notified on an
exception basis and are provided with a complete health
information summary that facilitates an informed and prompt
response.
The Personal Care Manager works with family, case
managers, visiting nurses and other clinical staff to educate
and inform the patient as to any changes in their health status.
Our interpersonal relationship with the patient developed
through our interactive video based TeleConsultation encourages
active patient participation.Patients
under the Independent Care™ program have round the clock access to our
TeleNursing Control Center in case they notice any changes in
their health condition. If there are any issues our staff will
follow proven, agreed protocols to triage the patients condition
and identify the appropriate actions, including the ability to
send help if needed.
