Our
Health@Home™ program
is designed for seniors and their families/caregivers who need
some additional assistance to live independently, but for whom assisted living is not desired,
justified or practical.HCT has a unique view and approach to managing care in
the home. We integrate clinical interactions and technology to
provide valuable services, including:

Individualized care
via personal two-way interactive video consultations

Remote monitoring of vital signs and
health factors from the home environment
- Medication reminders and education
- Coordination of care with current
caregiver network (physicians, nurses, aides, etc.)
- Additional concierge services to arrange
for personal care, transportation, laundry/cleaning, meals
and other needs
At HCT we believe in a personal, individual, patient
oriented approach. We assign Personal Care Managers to the patient
who are directly responsible for the daily monitoring and contact with
our patients. Our Personal Care Managers are qualified, licensed
and experienced home health care nurses who are dedicated to the quality
delivery of healthcare. The Personal Care Manager is an important
lifeline for the patient. A Personal Care Manager is available to
the patient 24 hours a day 7 days a week and are an important link to
outside resources. The Personal Care Manager can consolidate and
communicate information to physicians and can call on additional care
partners as needed for such things as transportation, pharmacy, and
assistance in the home.
 The
Personal Care Manager is also an important link to the family. The
Personal Care Manager can help update the family on the patient's current
health and can act as a source of helpful information. Many times
patients will not "bother" their families or are "embarrassed" about the
level of help they need but they will ask for help from their Personal
Care Manager.
HCT believes that a prompt response to a
problem or providing help to a patient can significantly improve
outcomes. Care can be dispatched quickly, ER visits can be
averted, problems can be resolved before they compound. |
